Are
you looking for an exciting new challenge where you can make a genuine
difference to people’s lives? How about joining a team at Erewash Voluntary
Action as a GP Link Worker for Erewash.
The
Erewash Primary Care Network (PCN) is a collaboration of the 13 GP practices in
Erewash working together and is committed to improving the care of patients in
Erewash.
The Erewash Primary Care Network is working with Erewash Voluntary Action to take forward Social Prescribing in the area. Social prescribing empowers people to take control of their health and wellbeing through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.
If you are a self-motivated, dedicated ‘people-person’ whose passion is to support and empower individuals to take control of their health and wellbeing, you could be just what we are looking for.

collaboratively with NHS and Voluntary Sector colleagues in Erewash, this could
be the job for you.
